Subvastus Approach to Knee Replacement

In my practice, I use the subvastus approach for most of my knee replacement surgeries. This advanced muscle-sparing technique is designed to improve recovery by working around the quadriceps muscle and tendon rather than cutting through them.

In a traditional knee replacement approach known as the medial parapatellar approach — still commonly used by the vast majority of surgeons — the quadriceps tendon is partially divided in order to access the knee joint. Other “minimally invasive” techniques, such as the mid-vastus approach, may involve cutting into the quadriceps muscle itself.

With the subvastus approach, the quadriceps tendon and muscle are preserved entirely. By avoiding unnecessary trauma to these important structures, patients tend to experience a smoother recovery and faster return of function after surgery.

The quadriceps muscle plays a critical role in walking, standing up from a chair, climbing stairs, balance, and overall knee function. Preserving the body’s natural anatomy is one of the reasons many patients recover more comfortably and regain mobility more quickly following surgery.
